  • General Liability Insurance in amounts not less than $300,000 per person and $500,000 per occurrence
  • The Department of Building and Zoning must be listed as the certificate holder

-A Bond in the amount of $25,000
-A Certificate of Liability Insurance with the limits of liability no less than three hundred thousand dollars ($300,000) for damages to a single person, and five hundred thousand dollars ($500,000) for one (1) occurrence.
-Individual name, and company name if applicable, must be listed identically on all documents. 
-This application must be notarized
