  • SAVE Affidavit (This form will need to be notarized and uploaded to the application portal)
  • E-Verify or Private Employer's Exemption Affidavit Pursuant to O.C.G.A.§ 36-60-6(d) (This form will need to be notarized and uploaded to the application portal)
  • Copy of Applicant's Identification
    • Passport, Military ID, or Valid Government Issued ID
    • Legal Permanent Residents: Valid Government Issued ID and a Permanent Resident Card or Employment Authorization Card
  • Emergency Contact Form - Insurance information is mandatory
  • Proof of Income (Submit one of the options below)
    • Prior Year's Federal Tax Return Transcript / Form 1040 Tax Return Transcript
    • Certified P & L Statement signed by company CFO / Controller
    • Signed and notarized letter from a C.P.A. firm (if applicable)
  • D.B.A. Registered Trade Name (if applicable)

  • Home Occupational Supplemental Form

    • Required if business will be operated from home

  • Copy of the Certificate of Incorporation

    • Required if business is a corporation or LLC (includes non-profits)

  • Copy of Professional State License

    • Required for: Attorneys, Physicians, CAP's, Engineers, Architects, Surveyors, Cosmetologists, etc.

  • Copy of health inspection report with grade and/or fire inspection report

    • Restaurants only

  • Copy of FOG (Fats, Oils, Greases) Compliance Inspection from DeKalb County Department of Watershed Management

    • Restaurants only

Due:

Licenses expire annually on December 31. Renewals must be filed by March 31 to avoid late fees.
