Class II A Checklist

In-person

• $60 license fee
• $35 fingerprint fee

Notary service is provided at MGC during the application process for no charge. Do not sign the Release of Information Authorization, Certification of Oath of Applicant, or Release of All Claims until turning in the completed packet in the presence of an MGC Notary. 

• Bingo Referral 
• 5 years of employment history (no gaps)
• 5 years of residence history (no gaps)
• Criminal History (disclose all charges as a juvenile and adult - regardless of it charges were dismissed, referred, sealed, and/or dropped)
• 2 Fingerprint Cards
• Employment Verification for the last 5 years
• Licensee Responsibility Agreement
• MGC Fair Credit Report Act
• NIGC Privacy Act
• Gaming Commission Mission Statement
• 2 Forms of ID
  
  • 1st can be a Driver's License, State ID, Military ID, or Tribal ID
  • 2nd must be a Social Security Card or Certified Birth Certificate (originals only) 
• Immigration Documents, if applicable
• Military DD214, if applicable
• 5 personal references and provide current address, telephone number, and email address

In-person

$50 renewal fee

Annually by the hire date of the licensee

Licensees must renew their license in-person at MGC. All information for renewal will be provided to the licensee before the expiration date of their license. 

Class III A Checklist

In-person

• $283 (in-state)
• $345 (out-of-state)
• $86 transfer fee (if applicable)
• $35 fingerprint fee
