Required Attachments: | A current NAIC biographical affidavit for each individual, member, officer, or owner of applicant and each person to be authorized to act under the registration. A detailed explanation of the company’s business plans for Montana including the marketing of its services. If applicant is using a d/b/a, provide authority to use such business name from the appropriate regulatory official from your state of domicile. Financial statements including a balance sheet (reporting positive net worth) and an income statement for the most recent complete calendar or fiscal year. Audited financial statements are to be submitted if available. |
