  • Certificate of Occupancy issued by the Department of Labor and Industry OR a copy of the letter of compliance or use and occupancy permit from the city or municipality where dealership is located
  • Pennsylvania Corporation Bureau Documentation authorizing entity to do business in the state
  • Approved fictitious name registration, if applicable
  • Franchise Agreement, if applicable
  • Hand Drawn Diagram of dealership
  • Copy of deed or lease agreement
  • Completed Inspection
  • Resume/Curriculum Vitae for each owner, partner, officer, or member for the past 5 years
  • Copy of telephone bill (must be landline) that lists the dealership's licensed name and proposed address
  • Zoning Documentation 
  • Criminal History Records Check or Exemption form for applicable individuals
  • Photographs of location that show overall view of showing display area and office, separation from any adjoining business or residences, inside of office showing telephone, desk, and lockable filing cabinet, area for display of vehicles to be sold separate from parking areas/other businesses with proper surface and adequate lighting, dealership sign permanently affixed or erected in outdoor display area showing licensed name, sign showing posted hours open
