$150 per system

Required Attachments:
  • Valid Arizona Registrar of Contractors License
  • Deficiency Report Agreement
  • For all systems other than Kitchen Hood Extinguishing Systems, submit one of the following certificates:
    • CSA level 4 or higher – Copy of ID Card
    • 5NICET 3 or higher – Copy of Certificate or ID Card
    • Certified Professional Engineer
  • For Kitchen Hood Extinguishing Systems, submit one of the following certificates:
    • CSA
    • NICET 3 or higher
    • Certified Professional Engineer

Registration Renewal

Form:

Business Certificate ​- Fire Protection Company (Renewal)

Agency Fee:

$150 per system

Due:

Triennially by the date of issuance or when the qualifications of certificate holders expire, whichever comes first.

Required Attachments:
  • Valid Arizona Registrar of Contractors License
  • Deficiency Report Agreement
