Required Attachments:
  • Negative Certificate of Criminal Record of the Individual or Representative of the Entity issued by the Puerto Rico Police, with less than 6 months of issue.
  • Gross Business Volume of the previous year (Income Contribution Form of the Corporation or Individual) and Municipal Patent.
  • A recent 2x2 photo of the Applicant or Entity Representative
  • Copy of the Certificate of Incorporation from the Department of State (in the case of domestic corporations), or Certificate to Do Business in Puerto Rico (in the case of foreign corporations), and Certificate of Existence and Compliance (“Good Standing”) (if it has been registered for more than two years).
  • ASUME debt certification (for individuals)
  • Copy of the Driver's License of the Individual or the Representative of the Entity
  • Bond issued by an insurance company (effective date must be closest to the date of making the request for Contractor Registration). It requires an Annual deposit based on gross revenue.
  • Copy of the Registry of Merchants of the Department of the Treasury (SURI)
  • Copy of water, electricity or telephone bill that shows physical address of the applicant
  • Declaration of Compliance with Consumer Responsibilities Form
