St. Croix Tribal Gaming Commission (WI)

Applications are not currently available online. Please call the gaming commission to request one. Follow the instructions below. 

1. Call the gaming commission to request an application. They will require, at minimum, the following information: 
   1. Name, Address, and Contact Person 
   2. Approximate monthly revenue
   3. Product or service to be supplied
   4. Copy of any proposed contract and thereafter, copies of any subsequent draft or update of the contract
   5. List of principal contact and employees doing work for the business on tribal property
2. Supply this information in the requested format. 
3. The gaming commission will forward you an application packet. 
4. Fill out and return the application packet, required documents, and fees to the gaming commission for review.

Renewal forms are mailed out prior to the expiration of the license.
