Required Attachments:
  • Surety Bond Form TC-450
  • Franchise Agreement, if applicable
  • Power of Attorney, if applicable
  • A passport-style photo of every owner, officer, member, and partner listed on the application 
  • Proof of attending an 8-hour orientation course (required of every owner, officer, member, and partner on application)
  • Utah State Sales Tax License
  • Photographs of business and sign
  • Standard FBI Fingerprint Card and Fingerprint Waiver form TC-465 for every owner, officer, member, and partner on the application
  • Plate Insurance Declaration Page, if applicable
  • Assumed Name Registration, if applicable
