An independent contractor not including Workmen’s Compensation on insurance policy is required to apply for an “Independent Contractor Affidavit of Exemption” from the Indiana State Workmen’s Compensation Board (800-824-2667). Form is available at the Building Department office.
Public liability insurance in the amount of not less than $100,000 for one person or $300,000 for one accident.
Property damage insurance in the amount of not less than $50,000.
Such workmen’s compensation, public liability and property damage insurance shall be kept in full force and effect during the entire licensed period and certificates to that effect annually.
Bond Requirements:
Surety bond in the sum of $5,000 payable to the City is required to be recorded with the: Lake County Recorder’s Office 2293 N. Main Street, Crown Point, IN 46307 (219) 755-3730.

City of Evansville/Vanderburgh County Contractor License

Insurance Requirements:
For Masters License, Commercial General Contractor’s License or an Unlimited Roofing License, an applicant shall, before being granted a license, make, execute and deliver to the Building Commission proof of general liability insurance in an amount not less than One Million Dollars ($1,000,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
Residential General Contractor, Limited Roofing Contractor, Wrecking Contractor or a Sign Erector Class B, shall before being granted a license, make, execute and deliver to the Building Commission proof of general liability insurance in an amount not less than Five Hundred Thousand Dollars ($500,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
For all other Sub-Contractor Licenses, an applicant shall, before being granted a license, make, execute and deliver to the Building Commission, proof of general liability insurance in an amount not less than Three Hundred Thousand Dollars ($300,000.00), with good and sufficient surety insuring all users of the licenses and indemnifying and holding harmless those persons sustaining loss or damages resulting from any and all work done under that license.
Each applicant for a Master’s or Contractor or Sub-Contractor License, shall, before being granted his/her license, provide proof of Worker’s Compensation coverage or an affidavit stating that by Indiana Law he/she is not required to have Worker’s Compensation Insurance.
This must be done within six (6) months of passing the exam and approval by the Licensing and Disciplinary Board.
Bond Requirements:
Each applicant for a Master’s or Contractor or Sub-Contractor License, shall, before being granted his/her license, make, execute and deliver to the Building Commission a Surety Bond in the amount of Twenty Five Thousand Dollars ($25,000.00) established in the applicant’s name on a form provided to the insurance company by the Building Commission.

Make sure you clearly state the license for which you wish to test.
Obtain three (3) letters of recommendation from reputable business or professional men, not related by blood or marriage, vouching for the applicant’s reputation as to honesty, integrity, and good character.
Fill out the middle of the Criminal Record History form and have it notoarized.
Electrical and HVAC recommendation letters must be on letterhead and notarized that prove at least four (4) years of experience in the field for which you are applying.
The application and letters must be submitted at one time to the Building Commission.
The applications will be submitted to the proper boards for approval to take the test. Once this is done, you will be contacted (via e-mail or phone) with approval to contact Ivy Tech (812)429-9852 to schedule and pay required fee for test you are taking.
Exam scores are reviewed by the Licensing and Disciplinary Board on the fourth Thursday of the month for acceptance or denial of license.
The applicant will be contacted by the Building Commission for the completion of the licensing process, which is the proper fee and providing proof of the required insurance and surety bond.
General Liability Insurance Certificate is required.

Must have $100,000 for each occurence of prperty damage, or must have combined single limit coverage which covers both bodily injury and property damage, minimum $500,000 per occurence.
