Due:
Expires after one year, then you may renew for either 1, 2, or 3 years at a time.
Applicants are given one year from the date the application is received to meet all requirements. If all requirements are not met within the one-year timeframe, the application and fees will be written off and the applicant will be required to submit a new application, documents, and fees.

Must be submitted by your insurance agent by email at application and at renewal of the license/registration. We cannot accept from the contractor. Certificates must be emailed from your insurance agent to insurance@lslbc.louisiana.gov.
Amendment
Instructions:
Amending the officers and/or directors can be done through the standard renewal process. However, this is considered a structure change and an additional fee will apply.
$120 per qualifying party exam/admin fee + $120 per qualifying party business and law fee.
Due:
If a qualifying party terminates employment with the licensee, the licensee must notify the board, in writing, within 30 days of the disassociation and another party must qualify within 60 days of the disassociation.

Submit the completed form called “Disassociation of a Qualifying Party” notifying the board of the disassociation of the qualifying party and the replacement qualifying party to be added.

If trade does not require a state license, a contractor must submit 3 notarized affidavits: 1 from a building supplier and 2 from licensed contractors in the applicable trade who are well-qualified to vouch for the quality of the contractor's work
There is no application to renew a contractor registration in Tangipahoa Parish. Mail or fax updated copies of the required documents and pay the annual fee to keep a registration current.

Lead Project Supervisors must successfully complete a recognized lead project supervisor training course; and have either one year of experience as an accredited lead based paint worker; or at least two years of experience in lead, asbestos, or environmental remediation work or in the building trades.

Workers who have received less than 16 hours of initial training must also submit proof of eight hours of training in 29 CFR 1926.62(l).
Submit a 1" x 1 1/4" full face photograph of the applicant issued by the recognized training provider for each of the disciplines the applicant is seeking accreditation.

Firms shall be responsible for mailing completed employee applications, along with the required attachments, photos, and fees to the address listed below, Attention: Special Services Division.
Owners/principals and employees must send in a fingerprint card, background check form and check in the amount of $39.25 made payable to the Louisiana Department of Public Safety for each background check. Also, employees that have been out of the industry for more than two (2) years or an employee adding a property protection endorsement for the first time must send in a fingerprint pack.

Insurance Requirements:
General Liability Insurance in a minimum amount of $100,000. If the insurance company is out of state it must say the policy is good in the state of Louisiana. The sole certificate holder must be the City of Sulphur, P.O. Box 1309, Sulphur, LA 70664. For contractors doing work in the City of Sulphur, the minimum General Liability Insurance is $300,000.
Bond Requirements:
$15,000 surety bond made favorable to the City of Sulphur stating the trade classification in which you are operating
